There were significant positive correlation between CYPⅡE1and MDA (p<0.01). and negstive correlation between CYPⅡE1 and SOD, GSH, VitE (P<0.05, P<0.05, P<0.01). conclusions Hepatocyte CYP ⅡE1 expression in nonalcoholic steatosis is prominent in hepatic aci- nar zone 3. Its expression resembles alcoholic fatty liver. There is significant correlation between CYPⅡE1 and lipid peroxidation.

目的研究非酒精性脂肪肝肝细胞色素P450ⅡE1表达及与氧化抗氧化的关系。方法用免疫组织化学的方法研究高脂饲料诱导的大鼠脂肪肝肝细胞色素P450ⅡE1的表达,同时检测肝组织中雨二醛(MDA)和超氧化物歧化酶(SOD)、谷胱甘肽(GSH)、维生素E(VitE)含量的变化,并分析其相关性。结果高脂饲料诱导的大鼠脂肪肝肝细胞色素P450ⅡE1在腺泡3区的表达增强,并从3区弥散至2区,与正常组相比,脂肪肝肝组织中MDA的含量显著升高(P<0.01).而SOD、GSH、VitE的含量显著下降(P值分别小于0.01、0.05、0.01)。相关分析表明,肝CYPHEI的表达与MDA呈非常显著的正相关(P<0.01)、而与SOD、GSH、VitE呈显著的负相关u值分别小于0.05、0.05、0.01)。结论非酒精性脂肪肝肝细胞色素P450ⅡE1在腺泡3区的表达增强,并呈弥散性的分布,与酒精性脂肪肝的表达相似,并可能与脂质过氧化反应有关。

Objective To explore the expression of hepatocyte cytochrome P450 Ⅱ E1 (CYP Ⅱ E1) in nonalcoholic steato-hepatitis. Methods CYP Ⅱ E1 were detected in liver tissue immunohistochemically by using specific antibodies directed against CYP2E1, and streptavidin - peroxidase (SP)method. Results The NASH rats showed increased CYP Ⅱ E1 immunostaining, especially in the perivenous region. Conclusion In NASH, the expression of CYP Ⅱ E1 is increased.

目的 探讨非酒精性脂肪性肝炎(NASH)肝组织内细胞色素P450 Ⅱ E1(CYP Ⅱ E1)的表达情况。方法 采用抗生蛋白链菌素-过氧化酶法(SP)检测NASH大鼠肝组织CYP Ⅱ E1的表达情况。同时常规苏木素伊红染色观察肝脏病理学变化。结果NASH模型组动物CYP ⅡE_1阳性细胞显著增加;且多在中央静脉周围,这与肝脂肪性变趋于一致。结论 NASH肝组织中存在着CYP Ⅱ E_1的高表达。

Objective To explore the correlation of nonalcoholic fatty liver(called "fatty liver" for short ) and insulin resistance with abnormal glucose metabolism. Methods The subjects were 48 patients with fatty liver, while the controls were 15 normal persons. Oral glucose tolerance test(OGTT) and insulin release test were performed; and the area under curve of serum glucose (AUCSG), area under curve of insulin(AUCIns) and insulin sensitivity index(AUCSG/ AUCIns) were calculated. Results The insulin level in fatty liver group was significantly higher than that in normal control group at 60 min, 120 min and 180 min after glucose intake(p < 0.01) with the postpond peak. The blood glucose level in the fatty liver group was significantly higher than that in normal cintrol group at all time intervals except at 180 min after glucose intake(p < 0.05). The AUCIns and AUCSG in the patient group were significantly higher than in the control group 209.56 vs 124.22, 24.72 vs 19.96, respectively(p < 0.01). The insulin sensitivity index in the patient group was signifi- cantly lower than that in the control group (P < 0.05). There is no significant difference in the insulin sensitivity index be- tween different degree of fatty liver (P > 0.05). Conclusion it is evident that patients with fatty liver have insulin resistance with abnormal glucose metabolism, which is not related with the extent of fat infiltration in the liver, suggesting that insulin resistance with abnormal glucose metbolism is a very important contributor to the pathogenesis of fatty liver.

目的探讨非酒精性脂肪肝(脂肪肝)与胰岛素抵抗及糖代谢异常之间的关系。方法对48树脂肪肝患者做胰岛素释放试验和葡萄糖耐量试验,计算胰岛素曲线下面积,血糖曲线下面积和胰岛素敏感性指标(血糖曲线下面积/胰岛素曲线下面积),并以不嗜酒的正常人作为对照组。结果脂肪肝组口服葡萄糖 60 min、 120 min和 180 min后胰岛素水平高于对照组且高峰后移;除 180 min外脂肪肝组的各时点的血糖水平显著高于对照组;脂肪肝组的胰岛素曲线下面积(mU·h~-1i·L~-1)和血糖曲线下面积(mmol·h~-1·L~-1)分别为209.56和2472,对照组分别为124.22和19.96,差异有显著性;脂肪肝组的胰岛素敏感性显著低于对照组;轻度脂肪肝与中、重度脂肪肝的胰岛素敏感性的差异无显著性。结论脂肪肝患者存在着明显的胰岛素抵抗及糖代谢异常,且与肝的脂肪浸润的严重程度无关,提示胰岛素抵抗及糖代谢异常在脂肪肝的发病机制中具有十分重要的作用。
