The distribution of ferritin and prealbumin in normal kidney and 27 renal cell carcinoma (RCC) was studied using ployclonal anti-ferritin and antiprealbumin antisera and immunohistochemical techniques, Serum ferriltin and prealbumin leves in 31 patients with RCC were also examined using radioimmunoassay and immunoturbidemetry.The normal adult kidney showed consistent localization of ferritin and prealbumin immunoreactivity in the proximal convelouted tubules.Ferritin was experssed in 25 of 27 RCC; prealbumin was expressed in 24 of 27 RCC.The immunoreactivety for both ferritin and prealbumin was comfined mainly to the cytoplasmic rim in the clear cell carcinoma, and to the cytoplasm in the granular cell variandt.The mean value of serum ferritin in patients with RCC significantly higher than that of normal subjects(P<0.001). But postoperative(one week afternephrectomy)serum levels didn't reduce to normal and the mean serum values of ferritin in patients with neoplasms of extrarenal origin were also elevated.Serum prealbumin concentration was significantly higher in patients with RCC compared to normal subjects(403.33*66.79mg/1 versus 316.10*24. 29mg/1,P<0. 001). Furthermore, there was also a signifecant increase in serum prealbumin levels with advancing tumour stage, as well as a signifecant decrease in serun prealbumin leves one with week after nephrectomy(P<0.05).Of the 21 patients with RCC undergone nephrectomy at least six months ago the serum levels of ferritin and prealbumin were below the upper limit of normal in 16 patients with no evidence of recurrence, but were elevated in 3 patients with distal metastases.In concluson, ferritin is a sensitive but not specific tumour markerfor RCC while prealbumin is both sensitive and specific as a biological tumour marker for RCC.

应用免疫组织化学技术观察了27例肾癌组织铁蛋白、前白蛋白的表达,并应用放射免疫分析技术和免疫透射比浊法对31例肾癌患者进行了血清铁蛋白及前白蛋白的检测。结果表明:正常肾脏组织仅近曲小管有铁蛋白及前白蛋白表达。27例肾癌组织铁蛋白表达阳性率为92.6%;前白蛋白表达阳性率为88.9%。透明细胞癌免疫着色主要在胞浆边缘,颗粒细胞癌主要在胞浆内。肾癌患者血清铁蛋白较正常显著升高(P<0.001),阳性率为83.3%。非肾源恶性肿瘤患者血清铁蛋白浓度亦显著升高(P<0.001)。肾癌患者血清铁与正常无显著差别(P>0.05)。肾癌患者血清前白蛋白较正常显著升高(P<0.01),阳性率为91.7%。术后一周降至正常。而非肾起源恶性肿瘤患者血清前白蛋白与正常无显著性差别(P>0.05)。肾癌患者血清铁蛋白和前白蛋白浓度随肿瘤分期增加而升高。16例术后无复发者血清铁蛋白和前白蛋白与正常无显著性差别(P>0.05)。3例有远处转移者均见血清铁蛋白和前白蛋白升高。本实验显示:铁蛋白作为肾癌生物学瘤标敏感性好,但特异性差。前白蛋白可作为肾癌诊断及随访敏感且特异的生物学瘤标。
