AIM To investigate the relationship between plasmic diamine oxidase (DAO) and gut injury in acute necrotizing pancreatitis (ANP). METHODS ANP was induced by injection of sodium taurocholate with trypsin into the pancreatic duct in 8 dogs during abdominal operation. Serum levels of amylase was determined, DAO activities of plasma and ileal mucosa were measured dynamically. RESULTS In ANP group, the levels of plasmic DAO increased significantly in the first ( mol/s ,0 025±0 006 vs 0 011±0 002) and the second day ( mol/s ,0 019±0 005 vs 0 012±0 003), but after the fourth day ( mol/s ,0 006±0 001 vs 0 014±0 002) it decreased. The levels of DAO in ileum were also lowered. Serum levels of amylase was increased 3-10 times. ANP induced damage of epithlial villi of the mucosa and bacterial translocation was found in liver, pancreas, kidney, lung, spleen and mesenteric lymph nodes of all animals. CONCLUSION Plasma DAO level might reflect the damage of intestinal mucosa in ANP, and bacterial translocation is the result of the gut injury.

Objective The safety of ERCP performed on patients with known pancreatic disorder was probed.Methods The mean concentrations of serum amylase at 4 and 24h after ERCP were measured in 117 cases with known pancreatic disease and in 114 without that disorder as controls.Meanwhile the clinical manifestations of acute pancreatitis were also observed. Results With pancreatic disease,the mean amylase levels at 4 and 24h rose to 292.4±319.6 amd 226.5±262.9U/L respectively,significantly higher than that prior to ERCP(180.7±106.6U/L, P <0.01) as well as that in the controls (252±235.2,187.8±218.3U/L, P <0.05).Acute pancreatitis was clinically established in 10 cases (8.5%) with significance ( P <0.05) when compared with the control ( 3.5% ).Conclusion It is indicated that the incidence of hyperamylasemia and acute pancreatitis following ERCP in patient with known pancreatic disease increases with significance,appropriate preventive measures should be taken.

目的为了探讨胰腺疾病患者ERCP安全性。方法观察了117例胰腺疾病患者ERCP术后4小时、24小时血清淀粉酶变化及急性胰腺炎发生情况,并与114例非胰腺疾病ERCP术后患者作了对比研究。结果117例胰腺疾病患者ERCP术后4小时、24小时血清淀粉酶水平分别为292.4±319.6U/L及226.5±262.9U/L,明显高于术前水平(180.7±106.4U/L,P<0.01),亦明显高于对照组相同时间水平(252.1±235.2及187.8±218.3U/L,P<0.05),其中10例患者发生急性胰腺炎(8.5%),亦明显高于对照组(3.5%,P<0.05)。结论有胰腺疾病基础患者ERCP术后发生高淀粉酶血症及急性胰腺炎机率较非胰腺疾病患者明显增高,应采取必要的预防措施。

Objective To investigate the intrapulmonary expressions of IL-1B and IL-18 mRNA, and to detect the correlation of such expressions with lung injury in experimental severe acute pancreatitis (SAP). Expressions of IL-1J3 and IL-18 mRNA were observed in the lungs of normal rats. Intrapulmonary expressions of IL-1B and IL-18 mRNA were increased greatly during taurocholate-induced SAP, with statistical significance compared with the NC group(P<0. 01). Conclusions In addition to TNFa, overproduction of IL-1B and IL-18 in the lungs might play an important role in the course of ALI and ARDS during the experimental SAP.

目的 观察实验性重症急性胰腺炎(severe acute pancreatitis,SAP)肺组织内IL-1β及IL-18mRNA的表达,并探讨其与肺损伤的关系。方法 SD大鼠32只,随机分4组:正常对照组、SAP6h组、SAP12h组、SAP18h组。采用5%牛磺胆酸钠(0.1ml/100g)胆胰管内逆行注射诱导大鼠SAP模型。血清淀粉酶采用HITACHI-7150型自动生化分析仪测定;半定量RT-PCR检测肺组织内IL-1β及IL-18mRNA的表达。结果 造模后各时间点血清淀粉酶水平显著升高,12h达到高峰,与正常对照组相比,均具有显著性差异(P<0.01)。正常肺组织内可见IL-1β及IL-18mRNA表达;SAP各组肺组织内IL-1β及IL-18 mRNA的表达明显增强,与正常对照组比较均有显著性差异(P<0.01)。结论 除TNF-α外,肺内生成过多的IL-1β及IL-18在SAP并发急性肺损伤(acute lung injury,ALI)及急性呼吸窘迫综合征(acute respiratory distress sysdrome,ARDS)进程中可能发挥重要的作用。
