Objective To explore a simple and effective method of induced sputum and cellular analysis,and to investigate airway inflammation features in children with acute asthma.Methods A method to induce sputum with inhaled hypertonic saline was used in 37 acute asthmatic childhood patients and 48 normal subjects(control group) for collecting the sputum.A portion of the sputum treated with dithiothreitol(DTT) was performed to count the total cell in the hemocytomer and the other did differential cell counts in the smear with Wright's stain and toluidine blue stain.Results In the asthmatic group,84% were successful in collecting the sputum and PEF value did not go down( P >0 05).The total cell counts were(11 2±8 9)×10 9/L in sputum from asthmatic subjects and (7 1±6 2)×10 9/L from the control subjects( P <0 05).The eosinophils and the mast cells of the asthmatic group were significantly higher than that of the control group( P <0 01) in the differential cell counts.Conclusion The analysis of the induced sputum and investigation of the airway inflammation was a safe and reliable method;eosinophils and mast cells were mainly effective cells in inflammatory response in asthma.

目的探讨简便有效的诱导痰液及细胞分析方法,了解儿童哮喘发作期气道炎症的特点。方法运用高渗盐水雾化吸入诱导痰液的方法,收集37例哮喘发作期患儿、48例正常儿童(对照组)痰液。一部分痰液经二硫苏糖醇(DTT)液化后,在血球计数板上行细胞总计数;另一部分痰液涂片经瑞氏、甲苯胺蓝染色行细胞分类计数。结果哮喘组84%取痰成功,诱导过程中呼气峰流速(PEF)未见下降(P>005)。痰液细胞总计数哮喘组为(112±89)×109/L,对照组为(71±62)×109/L(P<005)。嗜酸性粒细胞、肥大细胞在哮喘发作期气道中明显升高(P<001)。结论诱导痰液分析、评判哮喘患儿气道炎症是一种安全、可靠的新研究方法;嗜酸性细胞、肥大细胞为哮喘发作的主要效应细胞。

In order to understand the change and its clinical significance of nitric oxide (NO) terminal metabolite(NO3- / NO2-)contents in induced sputum in asthmatic children, the inhalation of 3% hypertonic saline aerosol was used to induce the sputum and, then, the sputum coughed out was degradated by adding 0. 1% dithiothreitoKDTT) solution and PBS buffer. Subsquently, NO3-/NO2- contents and eosinocyte cation protein(ECP) level were measured with nitratase reduction and EIA methods. Moreover, pulmonary ventilative functions were also assessed by portable spirometer. As a result, the sputum NO3-/NO2- contents in stable stage(19 cases),mild attack(16 cases) and moderate/severe attack(11 cases) groups showed to be significantly increased in comparison with control group(16 cases, all P<0. 05). In addition, there was also a significant difference in NO3-/NO2+ contents and ECP level among all different affected groups(all P<0. 05). On the other hand, it was also demonstrated that NO3-/NO2- and ECP contents in induced sputum in asthmatic children were found to be negatively correlated with the prediction baseline of 1 sec forced expiration volume (FEC 1.0%, r= -0. 512, P< 0. 01 )and there was evidently a positive correlation between NO3-/NO2- contents and ECP level in induced sputum(r = 0. 607,P<0. 01). Therefore , it was concluded that NO might play an important role in the onset and development of childhood asthma. The assessment of NO3-/NO2- content could reflect the changes of airway inflammation so as to be very helpful as a way of judging the severity of the disease and the effectiveness of clinical therapies in asthmatic children.

To investigate the clinical significance of measuring the IL 8 level in induced sputum of asthmatic children,the concentration of IL 8 in induced sputum of 41 cases with varied severity during different conditions of the patients was measured through ELISA,and the ventilated function of lung was determined through portable spirometer It was found that the level of IL 8 in induced sputum of asthmatic children was higher than those of the controls Among the asthmatic group,the levels of IL 8 in induced sputum were significantly different and those of the asthmatic children significantly correlated with the baseline FEV 1 0% It concludes that the concentration of IL 8 in induced sputum can reflect the changes of airway inflammation,and the measurement of this cytokine can be used as a means to assess the severity of asthma as well as for the guidance of effectiveness in clinical therapy in children
