The method consists of extracting the pheno-barbital from acidified plasma (1.0ml) with diethyl ether, evaporatingthe organic phase to dryness,dissolving the residue in 0.1ml of methanol-chloroform (1: 1), and spotting an aliquot of this solution on a thin layerof silica gel buffered with borate (pH 10), developing the plate withchloroform-acetone (8. 5: 1. 5), and scanning the plate in reflectance modeatλs 240nm,λR 420nm (the plate was moved at speed of 20mm/min undera slit of 0. 5×10mm, sensitivity of×1,and the chart paper speed was satat 20 mm/min). The sensitivity of the photometric recording was in-creased by approximately two times when phenobarbital was convertedto the salt by using the borax buffer solution (pH 10) instead of waterto prepare the plate. Down to 0.2μg/spot of phenobarbital was determinedand linear calibration curve obtained. Mean recoveries obtained fromspiked plasma samples were 99.49%±1.01 (Sx). The method may be applied to toxicologic assay, clinicopharmacologicalor pharmacokinetic investigation for the determination of level of pheno-barbital.

Matrix-entrapment is one of the major methods of enzyme-immobilization. In order to overcome its shortcomings, such as low amount of entrapment, low reactivity because of high polymer content and low diffusion, as well as irregular shaping, the entrapment of urease by crosslinked polyac-rylamide had been investigated and spherical immobilized urease had been prepared with following properities. High amount of immobilized enzyme (1 part of enzyme to 1 part of polymer) and about 50% relative activity of the immobilized urease. Reactivity towards urea hydrolysis was 120mg/ ml for two hours at 37℃. The stability of the immobilized urease was increased in comparison to the free urease. When stored at 4℃, the loss of initial activity was 8% after 66 days, 22% after 106 days and at 37℃ the loss was 18% after 11 days. The spherical shape of the beads improved the mechanical strength and enhanced the column velocity. No leakage of the immobilized urease could be detected. The above properties render the immobilized urease suitable for active material as artificial kidney.

本文以交联聚丙烯酰胺对尿酶包埋进行了研究,制备出了球形固定化尿酶,其含酶量高(1份酶对1份聚合物),相对活性为50%左右,对尿素的分解活性:120m/ml(固定化尿酶,37℃,2小时),提高了稳定性,在4℃下,66天后活性降低8%,106天后活性降低22%,37℃下11天后活性降低18%,并且不泄漏酶。基于以上特点,该固定化尿酶为吸附型人工肾提供了一个较合适的活性材料。

本文测定了20例慢性肾功能衰竭(慢性肾衰)维持血液透析(血透)患者血透前及血透24、48小时后的凝血像。结果表明:在血透前,各项指标的改变与肾衰非透析病人相似,显示血透病人亦存在不同程度的高凝状态。透析24小时后,高凝指标的变化更为显著,尤以ⅧR:Ag 增高、AT-Ⅲ降低、TEG-γ值减少以及 TEG-ma 值增高最为明显。在血小板的检查中,透析前患者有血小板功能不良,但透析后并无相应改善。上述结果说明,透析后病人的出、凝血机理更为紊乱,这可能与注射肝素和所用的人工肾类型有关。提示目前我们进行的透析技术尚待改进。定时检测患者的凝血像,有利于及时制定相应的治疗措施,并防止血透并发症的发生。
