To explore the changes of higher-order aberration (HOA) with age, and to evaluate the symmetry of the aberrations between right and left eyes. Higher-order aberration of 69 normal eyes (35 persons) were examined across a 6.0 mm pupil using Zywave which based on the theory of Hartmann-Shack. All subjects were divided into three groups according their age: youth group (age < 35 years); middle-aged adults (age 35 - 45 years); old adults (age > 54 years). The root mean square (RMS) values of higher-order aberrations of the three groups were calculated and univariate analysis of variance was performed. The association between ocular HOA and age was analyzed, as well as the aberration symmetry between right and left eyes using Zernike terms. Higher-order aberrations, spherical aberration (SA), the third-order aberrations, and the forth-order aberrations were associated with age (P < 0.05); but coma and the fifth-order aberrations were not associated with age (P > 0.05). Eight of the fifteen Zernike terms were significantly correlated between right and left eyes(P < 0.05). [Conclusion] Higher-order aberration, especially spherical aberration increased with age. A moderate degree of mirror symmetry existed between right and left eyes.K

【目的】研究人眼高阶像差(higher-orderaberration,HOA)随年龄增长的变化规律以及HOA在左右眼的对称性。【方法】用一基于Hartmann-Shack原理的Zywave波前像差仪测量了35例正常人69眼的HOA,评估时所取的瞳孔直径为6mm。根据测试对象年龄的不同分为3组:青年组(<35岁),中年组(35岁~54岁),老年组(>54岁),计算各组高阶像差的均方根(rootmeansquare,RMS)值,并对3组进行单因素方差分析,对HOA与年龄作相关分析,研究高阶像差随年龄的变化规律;同时对左右眼各Zernike系数作相关分析,以研究高阶像差在左右眼的对称性。【结果】总体高阶像差(HOA)、球差、三阶与四阶像差与年龄均有相关关系(P<0.05),彗差与五阶像差与年龄的关联性不大(P>0.05);左右眼的15项Zernike系数中有8项存在关联性(P<0.05)。【结论】高阶像差随年龄的增长而增加,左右眼的高阶像差存在一定程度的对称性。

Objective To analyze the distribution of the higher-order aberrations in the population of China with myopia and astigmatism,and to determine whether the pupil size,degree of myopia and astigmatism of higher-order aberrations.Methods One hundred and three myopic astigmatism eyes were evaluated before refractive surgery by using a Hartmann-Shack method.The root mean square(RMS)values of higher-order aberrations for each subject with pupil diameters of 5.0 mm and 6.0 mm were calculated.The subjects were divided into following groups:high myopia(>-6.00 D),medium myopia(-3.00~-6.00 D)and low myopia(≤-3.00 D),high astigmatism(≥0.75 D)and low astigmatism(<0.75 D).Results Higher-order aberrations were increased significantly with increasing pupil size.The RMS values were decreased from 3rd to 5th order aberrations with pupil diameters of 6.0 mm in the same eye.Spherical aberration and coma aberration were affected by high myopia;low and moderate myopia did not effect higher order aberrations.Astigmatism only affected the coma aberration,but not spherical aberration.ConclusionThe aberrations increase with the pupil diameters,the increasing of refractive error such as myopia and astigmatism.

目的了解我国近视散光患者眼高阶像差的分布,瞳孔直径、近视、散光度数对高阶像差的影响。方法像差数据由哈特曼-夏克Hartmann-Shack原理设计的Zywave波前像差仪对不同瞳孔直径的近视眼进行测量,共对103眼准分子激光手术前的近视散光情况进行检查,提取当瞳孔直径为5.0 mm、6.0 mm时高阶像差的均方根(RMS)值,根据近视和散光的度数高低分组对比研究分析。结果随着瞳孔的增大,高阶像差相应增加。6.0 mm瞳孔下,同一眼内3~5阶像差的RMS值呈递减趋势。在低度和中度近视患者中,各高阶像差和屈光度无明显相关性;在高度近视患者中,高阶像差随着近视度数的增加而增加。彗差和球差均与屈光度有明显的相关性。高阶像差随着散光度数的增加而增加。彗差和散光度有明显的相关性,但球差和散光度无明显的相关性。结论高阶像差在个体中存在较大差异,瞳孔增大时球差和慧差比率增加,散光影响球差和慧差,瞳孔直径、近视和散光程度对高阶像差有明显的影响。

There was no significant difference (P>0.05) in all refractive results among low myopic, moderate myopic and high myopic eyes. The rate of matching in spherical refraction between measurement with Zywave aberrometer and cycloplegia autorefraction decreased with the increasing degrees of myopia. However this matching rate in cylinder increased with the increasing degrees of myopia.Conclusion Zywave aberrometer is highly accurate in refractive measurement.

目的评价Zywave波前像差仪测量近视眼屈光不正的准确性。方法对56例行屈光不正矫正术的患者(106只眼)分别用电脑验光(睫状肌麻痹和非睫状肌麻痹下)、显然验光、Zywave波前像差仪法测量眼屈光不正,并将患者按屈光不正度数分为三组:低度近视组(-0.50~-3.00D),中度近视组(-3.25~-6.00D),高度近视组(-6.25D~)。对测量的结果进行单因素方差分析(ANO-VA)。结果四种验光方法测量的验光结果(球镜、柱镜、等效球镜)除低度近视组电脑验光和睫状肌麻痹状态下验光的球镜度数(P=0.029)及等效球镜度数之间的比较差异有显著性(P=0.024)外,余在低度近视组、中度近视组及高度近视组两两之间比较差异均无显著性(ANOVA,P>0.05)。Zy-wave波前像差仪和显然验光及睫状肌麻痹状态下验光测量结果比较,随屈光不正度数增加,球镜相符率降低,柱镜相符率升高。结论Zywave波前像差仪测量屈光不正有较高的准确性,可以很好地用于Zyoptix系统个体化切削治疗使用,但与显然验光和睫状肌麻痹状态下验光比较仍有差异,可以作为以上两种验光方法的参考和补充。
