In response to hormonal stimula-tion (pituitary gonadotropin or pro-gesterone) in vitro,microvilli retra-cted gradually,accompanied by nu-clear and cytoplasmic activities of the oocyte during maturation division.Perivitelline space appeared at the animal pole around 9 hours after hormonal treatment (18±1℃),and extended to vegetal pole thence.Whe-rever the perivitelline space appeared,Vitelline membrane was formed throughtransformation from the zona pellu-cida and became separable from the plasma membrane of the developing oocyte.The perivitelline space en-circled the entire oocyte surface at about the time when the metaphase spindle of the first meiotic division established (13-14 hours).Accor-dingly,the newly formed vitelline membrane,transparent and tough,be-come totally removable from the ma-tured oocyte.The perivitelline space increased unequally,more wider at the animal pole and the vitelline membrane became more tough.It was interesting to find that whether the oocytes was normally ovulated or remained in the follicle after 20 hours hormonal treatment,the above men-tioned changes could be observed,only if the oocytes resumed their me-iotic division and arrested at the me-taphase of the second meiotic division.It is now clear that vitelline mem- ??brane is derived from zona pellucida in the course of oocyte maturation (not ovulation).It is well known that fertilization membrane is trans-formed from vitelline membrane short-ly after fertilization.Thus,one must view the relation among the zona pellucida,vitelline membrane and fert-ilization membrane during the de-velopment of the egg from their con-tinuity and phasic speciality. At the same time,there are also changes in the ovarian membrane.Microfilaments,measuring 50-70 A in diameter,were found to be scat-tered first randomly in surface epi-thelium cells,parallal to the surface of the oocyte,at the beginning of hor-monal treatment (3 hours); but or-ganized into bundles after 12 hours of treatment.The somewhat straight basement membrane at the beginning changed into a wave-like structure 12 hours afterwards (at or about thetime of ovulation),and the surface epithelium cells and their nuclei had some corresponding contortions.Mo-reover,dissociation of the fibroblasts and the collagen fibers within the connective tissue layer were also ob-served.It was found that the ma-tured oocytes deprived previously of ovarian membrane remained within the follicle 22 hours after hormone treatment without ovulation.It sug-gests that it is the ovarian membr-ane,not the follicular epithelium,which is responsible for the oocyte ovulation.Since ovulation can occur from isolated follicles,in vitro,with-out the participation of non-fol-licular ovarian smooth muscle or the internal follicular fluid pressure,it must be associated with some non-muscular contractile mechanism pre-sent within the ovarian membrane itself.

Water—soluble contrast agent, myelography showed that the majority of ILDH patients have had an irregularly marginated defects or complete block, and the widenning of the anterior space between VWD and PLL. CTM may best confirm the presence of ILDH material in some cases. As soon as ILDH is diagnosed or suspected a prompt surgical decompression is an absolute requirement. The results of operation were generally favorable as long as the surgical procedures were carefully managed.

硬膜内腰椎间盘突出症临床上少见,术前诊断困难。本文报告6例,其中5例发生于L_(4-5),1例L_(3-4)。6例中2例有椎间盘手术史。术中均发现腹侧硬膜与后纵韧带紧密粘连、无法分离。水溶性非离子碘造影剂脊髓造影显示位于椎间隙平面的全梗阻、不全梗阻(本组各3例),根据梗阻断端边缘不规则或呈毛刷状。且有梗阻头、尾侧的硬膜囊前间隙增宽的特点,即可明确诊断。CTM(脊髓造影CT扫描)于梗阻部位扫描,能显示突出物与硬膜、蛛网膜下腔、神经根等的关系。本组5例术前已明确诊断,可能与水溶性造影剂脊髓造影及CTM联合应用有关。硬膜内突出一经确诊,应尽早手术,手术效果满意。
