Abstract To assess the feasibility and reliability of multiplane transesophageal echocadiography (MTEE) in measuring the area of atriai septal defect (ASD),MTEE was performed in 23 cases with ASD.After the longest diameter of ASD was imaged,the phased array was rotated every 10°through a 0°- 180°arc to obtain a complete recording of 18 diameters of ASD. Abstract To assess the feasibility and reliability of multiplane transesophageal echocadiography (MTEE) in measuring the area of atriai septal defect (ASD),MTEE was performed in 23 cases with ASD.After the longest diameter of ASD was imaged,the phased array was rotated every 10°through a 0°- 180°arc to obtain a complete recording of 18 diameters of ASD. Using a soft ware program and ECG gating,the area of ASD was recontructed and measured at 7 time points of one cardiac cycle.The result showed that all ASD areas measured at 7 time points by MTEE correlated well with that measured at operation (r= 0. 820. 92) and the best correlation obtained was between ASD area measured at the peak of P wave by MTEE and during operation (r=0. 92). Thus,MTEE offers a new and reliable method for measuring ASD area.

为了探讨新型多平面经食管超声心动图(MTEE)测量房间隔缺损(ASD)面积的可行性和可靠性,在23例ASD患者中进行了MTEE检查。利用心电图门控于心动周期的P波顶点、P-R段、R波顶点、J点、T波起点、T波顶点、T波终点等7个时相,以MTEE技术每隔10°扫查角度测量ASD直径,由此绘出心动周期每一时相的ASD图形,并计算其面积,然后与手术测量的ASD面积进行比较。结果显示:(1)ASD面积在心动周期中呈动态改变;(2)MTEE于P波顶点测量的ASD面积与手术测值相关最佳(r=0.92),余心动周期时相MTEE与手术测值亦相关良好(r=0.82~0.88),表明MTEE为ASD面积的测量提供了可靠的新方法。

The result showed that the diagnostic accuracy of TTE and MTEE was 68. 4% and 100% respectively,when compared with surgical findings. MTEE was highly superior to TTE in detecting small ostium secundum ASD, sinus venous ASD and multiple ASD as well as associated partial anomalous pulmonary venous drainage. The maximal diameter of ASD measured by TEE correlated moderately with that measured at operation, while the same parameter measured by MTEE correlated well with that measured at operation(r= 0. 75 and 0. 95 respectively, P<0. 001).It is conclueded that MTEE offers a new and accurate technique for qualitative and quantitative diagnosis of ASD.

目的为了继续探讨经导管关闭继发孔房间隔缺损(ASD)的临床应用。方法继用Sideris的纽扣式补片关闭ASD后,我们又尝试采用CardioSEAL装置关闭ASD4例。此补片为蚌状夹式双伞型闭合器装置,有12、17、23、28、33、40等型号。根据球囊测量ASD最大伸展直径选择补片,其比例为1.7~2.2:1。结果4例均为一次关闭成功,其中一例为两个房缺孔。目前,所有病例仍在追踪随访中,暂未发现有补片移位,支架断裂,心脏其它结构受影响或残余分流等并发症。结论本方法操作较易掌握,补片不易移位。但由于仅适用小于20mm的ASD病例,且价格较贵。所以,手术适应证的选择要掌握好,避兔增加病人的痛苦。此方法仍在临床试用阶段,有待进一步论证其应用前景。
