Palatine tonsils from patients with recurrent tonsillitis (N = 31) and fetuses (N = 8) were used for study by SEM and TEM. The method of immunogold-silver staining ( IGSS ) was usedto study the distribution of T cell subsets in crypt epithelium under light microscope. Results showed that there were three types of microcrypt opening on the surface of the crypt epithelium. They may serve as orifices for the inter-epithelial passage of wandering cells and entrances for exogenous antigens and microorganisms. Some microcrypt openings were covered by microfold cell ( MC ) . Three types of MC were observed. With the advance of age and repeated tonsillitis MC may decrease in number. In cryptic epithelium, most of infiltrating mononuclear cells were OKT~+_3 cells with OKT_4 cells predominating, while only a few scattered T cells were OKTs cells.

采用扫描电镜、透射电镜及光镜免疫金银法观察了31例慢性扁桃体炎患者和8例胎儿的扁桃体隐窝上皮结构及其中细胞的分布。电镜观察发现隐窝上皮表面存在三种类型微隐窝开口,其腔内有浸润细胞、异物及细菌。有三种特化上皮细胞(M细胞)覆盖于Ⅲ型微隐窝开口处,其结构与肠道M细胞相似,其数量随扁桃体炎的反复发作而减少。形态表明微隐窝是浸润细胞和外来抗原的出入口。M细胞与抗原的摄取及传递有关。光镜免疫金银法观察证明上皮浸润细胞中多数为OKT_s~+细胞,其中OKT_4~+者又占多数而OKT_s~+细胞较少,这些细胞是隐窝上皮参于免疫应答的结构基础。

By using of electron microscopy the palatine tonsils obtained from patients with recurrent chronic tonsillitis(children and adults)and aborted fetuses aged 5-7.5 months were studied in this paper.In children and adults, there are three kinds of blood vessels in the tonsillar crypt epithelium.They are fenestrated capillaries,high endothelial venules and intermediate type blood vessels composed of both flat and cuboidal endothelial cells.Many plasma cells and lymphocytes were seen around these blood vessels, Lymphocytes penetrating through the wall of the vessels were observed.Many capillaries distribute beneath the crypt epithelium in fetuses.Some of them lay closely against the basal lamina of crypt epithelium and grow toward the epithelium in embryonic month 7.The results suggest that these blood vessels may serve as the route for lymphocyte recirculation in crypt epithelium and immunoglobulin release into blood flow, These blood vessels in the crypt epithelium may be derived from the blood vessels beneath the epithelium and enter into the epithelium with infiltrating cells.

用透射电镜观察慢性扁桃体炎患者和胎儿的腭扁桃体隐窝上皮内血管的结构及分布。发现儿童及成人腭扁桃体隐窝上皮内存在着有孔毛细血管、毛细血管后微静脉及由扁平内皮与矮立方内皮构成的过渡型血管。这3种血管周围有淋巴细胞、浆细胞等聚集,可见白细胞贴附或穿越血管壁。在胎儿扁桃体隐窝上皮内有白细胞等浸润,但未见血管,隐窝上皮基膜下组织中毛细血管丰富,7月龄胎儿可见毛细血管突向上皮内。本研究结果表明,扁桃体隐窝上皮中的血管构成了淋巴细胞再循环及抗体等物质进出血液的途径。隐窝上皮内血管可能是上皮下血管随浸润细胞一起长人上皮的。

目的 :探讨慢性扁桃体炎患儿扁桃体隐窝上皮的变化。方法 :将摘除的 2 30例扁桃体均以 10 %福尔马林固定 ,常规石蜡包埋切片 ,HE染色 ,并行抗隐窝上皮角质蛋白 (CK)免疫组化检测 ,观察小儿慢性扁桃体炎症变化。结果 :2 30例扁桃体隐窝上皮明显角化 2 0 9例 (90 .9% ) ,未见角化 2 1例 (9.1% ) ;3例隐窝底部有霉菌菌丝和细菌 ;扁桃体隐窝上皮抗广谱和高分子 CK均呈阳性反应 ,全组隐窝内角化物抗广谱 CK亦均呈阳性反应。结论 :小儿扁桃体每次炎症病变 ,都使扁桃体上皮进行性破坏 ,扁桃体产生免疫球蛋白的物质减少 ,因而不能有效发挥保护作用 ,而细菌、病毒等物质却可顺利通过化生性上皮向上皮下侵犯 ,这种周而复始的恶性循环 ,只有施行扁桃体切除术方能阻断
