In order to study chronic occupational hazards of contacting low dose dangerous factors and find effective countermeasures of prevention, occupational health surveillance data were analyzed. The exposed dose of two cross-sectional in the surveillance, the data of peripheral blood cell, occupational disease rate, tumor rate were used as indication. People contacting low dose dangerous factors were as observation groups and people not contacting the factors were as control group. Capturing and re-capturing were selected to compare differences between the two groups. The results showed the ACPR of mix-benzene and X-ray's WBC decrease in observation groups were 35.90% and 17.91%, RR were 1.69% and 3.84% respectively. The ACPR in control group were 4.67%. The ACPR of occupational diseases and tumors in observation groups were 5.75% and 2.99% respectively. RR of occupational diseases were 1.9%. No occupational disease and tumor cases were found in the control group. ACPR and RR of peripheral blood abnormality, occupational disease and tumor were mix-benzene>X-ray>control. The workers whose WBC was decreased must leave from their positions in time so as to block occupational disease occurrence. Effective prevention must be strengthened to reduce X-ray exposure dose because injury of X-ray on human body was chronic.

通过职业性健康监护结局的分析 ,观察有害因素低剂量下慢性职业危害特点 ,为采取相应临护对策提供医学依据。取经常性监测与监护中两个断面的暴露剂量与外周血异常 ,职业病与肿瘤发生资料作观察组的观察指标 ,以周期非有害作业人员作对照 ,用俘获再俘获法直接比较。观察组混苯与X射线的WBC下降的ACPR分别为 35 .9%、17.91% ,RR为 7.6 9、3 .84,对照组ACPR为 4.6 7% ;职业病与肿瘤的ACPR为 5 .75 %、2 .99% ,职业病RR为 1.9。对照组未见发生。外周血异常和职业病与肿瘤的ACPR与RR :混苯 >X射线 >对照组。混苯作业外周血下降及时调换工种 ,利于阻断职业病发生或加重 ,X射线组慢性损伤潜伏期较长 ,应加强有效防护

Objective To explore the pattern of pollutants changes and diseases occurrence in railway extralong or long tunnel and to provide scientific data for establishment of health standard in that kind of tunnel .Methods The tunnel working environment was assessed and 72 tunnel workers and 59 non-tunnel workers were studied. Results The all dust concentration and noise level all exceeded the national standard. In addition, the level of dust, nitrogen oxides, sulphur dioxide, noise, etc. in the tunnel have increased year by year. The health status of the tunnel workers showed that hearing impairments, abnormal electrocardiogram and changes of lung markings tended to be increased and aggravated year by year, on the contrary, the count of erythrocytes, leucocytes, lymphocytes and neutrophils tended to be descended year after year. Conclusion The occupational hazards of the railroad long tunnel on workers might be of chronic in nature.

目的探讨铁路特长、长隧道内污染物和疾病的发生发展规律,为国内特长、长隧道卫生标准的制定提供科学依据。方法对某长隧道作业环境进行调查,同时对72名隧道作业工人和59名对照进行健康监护。结果隧道内空气中粉尘、噪声起过国家卫生标准,且粉尘、噪声、NOx、SO2污染日趋严重,氧含量逐年升高,健康危害以听力损害最严重,其次为心电图异常逐年上升,肺纹理改变逐年加重,红细胞、白细胞、淋巴细胞、中性粒细胞计数逐年下降。结论 某铁路长隧道对作业人员的危害表现为慢性职业危害
